How to run a client intake and write it up

A client intake runs from rapport through the health screen and medical history, lifestyle, baseline measurements and goals, and ends by recapping what you learned. Write it up on one page: goals in the client's words, the health screen exactly as answered with any yes marked for clearance, where they are starting, the life around their training, what could get in the way and what already works.

What should the health screen cover?

The PAR-Q+ asks about known heart conditions, chest discomfort, dizziness or loss of consciousness, and conditions or medications that affect exercise readiness. A yes means medical clearance or guidance before starting or before progressing intensity. When uncertain, err toward clearance.

Why write 'not recorded' instead of 'none'?

Because an intake that did not ask is not a clean bill of health. A summary that says 'none' where nobody asked hides exactly the question the first program needs answered.

How Fenlio keeps what the intake said

Paste or drop the intake and Fenlio proposes the client's record from it: name, contact details, and the injuries, allergies, medical notes, equipment and preferences it found, each in the client's own words. You confirm what is right before anything is saved, and Fen writes the intake summary from the same record.

Questions

Should the intake summary go to the client?
No. It is your own file, read before the first program and whenever a decision needs the full picture.
