Pre-consult questionnaire template for coaches
A pre-consult questionnaire is the one-page note a coach sends a lead before a consult call.
What goes in it
It plays back what you heard about their goal in their own words, names where they are likely stuck, lists the questions you will ask to confirm it, and suggests one possible first step from your method. It shows you listened and lets the consult start at the useful part.
One page, roughly 300 words.
- 1
Title
Names the person and their goal.
- 2
What we heard
Their situation and goal, in their words where your notes record them.
- 3
Where it is likely stuck
Each item: what is in the way on the first line, what it is costing them on the next, only as your notes state it.
- 4
What we will ask
Questions that would confirm or rule out each issue.
- 5
A possible first step
One concrete starting move from your method.
When to send it
When to send it
- After a lead fills in your application or enquiry form.
- Before a second call with a lead who is close to deciding.
- When a lead's partner or family will help them decide.
Common mistakes
- Guessing what their struggle is costing them when they never said.
- Turning it into a sales page.
- Answering every question yourself instead of listing what you need to ask.
Why it works
Gong's study of 67,149 sales meetings found top performers answer an objection with a question far more often than average reps, and pause longer before replying.

Let Fen write it
- 1
Bring the client's record
Their intake, check-ins and messages, pasted, dropped or imported into that client's record.
- 2
It can't make up a number
Every weight, date and figure Fen writes is checked against the client's own words, and a load or macro target is computed from your rules. One with no source is flagged, never guessed.
- 3
You decide what goes out
At default settings nothing reaches a client until you press. Email replies go out from your own Gmail when you press Approve and send.
Questions
- Why send something before a consult?
- It shows you listened, gives the lead a chance to correct you, and lets the call start at the useful part instead of the recap.
- How long should it be?
- One page, about 300 words. Long enough to be specific, short enough to read before the call.
